Plan Transition Rules (EMR)
Orion → Lyra: When baby turns 3 months, reduce from $295 to $225. Change in EMR effective the 1st of the month after they turn 3 months.
Lyra → Sirius: When child turns 1 year, reduce from $225 to $175. Change in EMR effective the 1st of the month after they turn 1 year.
Sirius → Pegasus: When child turns 4 years, reduce from $175 to $150. Change in EMR effective the 1st of the month after they turn 4 years.
Pegasus → Polaris: When young adult turns 19, reduce from $150 to $100. Change in EMR effective the 1st of the month after they turn 19.
